Life Changers services are a combination of transitional housing options with multiple mental health intervention services provided to individuals (adults, children and/or adolescents) and their families that help them live more stable, productive lives. Program participants are provided necessary mental health services while residing within one of several housing options either provided by or assisted by YES Behavioral Health, Inc. Caseworkers and housing search professionals from our staff work with the families to formulate individualized plans to help them move into permanent, affordable housing.
A Range of Housing Models
While there may not be a single perfect model, there are a number of housing models for the Life Changers Program that include:
- Emergency shelter beds
- Scattered-site apartment units
- Rent-subsidized apartments leased in the open market; and
- Long-term set-asides of units within privately owned buildings

The Mission
To provide an array of intensive mental health services, crisis management, training and support to enable individuals with significant functional limitations and/or dysfunction to achieve and maintain community stability and independence in the most appropriate, least restrictive environment.
Who is Eligible?
While supportive housing is a useful intervention for a wide range of people who are homeless or at risk of homelessness, Life Changers focuses on working with our partners to create permanent supportive housing opportunities for adults, youth/young adults, and families with children who:
- have chronic health conditions such as mental illness, and/or substance use issues
- have been homeless for long periods of time or have experienced repeated stays in the streets, emergency shelters, or other temporary settings, often cycling between homelessness and hospitals, jails, prisons, or other emergency systems
